What are Uterine Fibroids?

If you have just been told you have fibroids, or you are still waiting to find out, it is completely natural to feel anxious or worried. The first thing to know is that fibroids are almost always benign, meaning they are not cancerous, and in many cases they can be managed without surgery.

Uterine fibroids are growths that develop in or around the uterus. They are extremely common. Around one in two women will develop fibroids by the age of 50, though many will never know they have them because they cause no symptoms at all.

For those who do experience symptoms, fibroids can genuinely affect daily life, causing heavy periods, pelvic pain, pressure and fatigue. If that sounds like you, getting proper advice from a specialist is the most important next step. Fibroids are oestrogen-dependent, which means they are uncommon before puberty and tend to shrink naturally after the menopause. They are more prevalent in women of Afro-Caribbean heritage.

Types of Fibroids

Where Do Fibroids Grow?

Fibroids are classified by their location within the uterus. Understanding which type you have helps determine the most appropriate treatment, and your consultant will explain this clearly at your appointment.

Submucous Fibroids

These grow inwards into the uterine cavity. They are most likely to cause heavy bleeding and period problems. Often the most symptomatic type, but they respond very well to hysteroscopic surgery, which is usually performed as a day case.

Intramural Fibroids

These grow within the muscular wall of the uterus and are the most common type. Depending on their size and number, they can cause heavy periods, pelvic pain and pressure. They are typically treated with laparoscopic myomectomy or uterine artery embolisation.

Subserous Fibroids

These grow outwards from the outer surface of the uterus and are more likely to cause pressure symptoms such as bladder or bowel changes, abdominal bloating and a feeling of heaviness. They can grow quite large before causing noticeable symptoms.

Fibroid Treatment at Ovara Health

Watchful Waiting and Monitoring

Many fibroids do not require any treatment at all, particularly if they are small and not causing significant symptoms. Regular ultrasound monitoring keeps track of any changes over time. Your consultant will tell you clearly what to look out for and when further discussion might be needed.

Non-invasive, low risk

Medical Management

Hormonal treatments including progesterone-releasing coils (Mirena), oral hormonal medications and GnRH analogues can help to control heavy bleeding and reduce fibroid size. Your consultant will advise on the right option based on your symptoms, your fertility plans and your overall health.

Effective for symptom control

Hysteroscopic Myomectomy

A minimally invasive procedure to remove submucous fibroids through the cervix using a slender camera called a hysteroscope. There are no incisions and it is typically performed as a day case under a short anaesthetic. It is highly effective at resolving the heavy bleeding that these fibroids cause.

Laparoscopic Myomectomy

Keyhole surgery to remove intramural or subserous fibroids while keeping the uterus intact. This is the preferred surgical option for women who want to preserve their fertility. Our consultants have advanced laparoscopic skills and access to robotic-assisted techniques where appropriate.

Fertility-preserving surgery

Uterine Artery Embolisation

A non-surgical procedure that works by blocking the blood supply to fibroids, causing them to shrink. Performed by an interventional radiologist, it is particularly well-suited to women with large or multiple fibroids who would prefer to avoid surgery. Usually carried out as a day case or short overnight stay.

Endometrial Ablation

A procedure that removes or destroys the uterine lining. It is suitable for women who experience heavy periods related to smaller submucosal fibroids and who have completed their family. A quick day case procedure with a fast recovery, and it can significantly reduce or stop heavy bleeding.

Most women with fibroids go on to conceive naturally and have healthy pregnancies without any intervention at all. That said, many women with fibroids have no symptoms at all and only find out through a routine scan. The only reliable way to confirm a diagnosis is a pelvic ultrasound, which we can arrange at your initial appointment.
No. Many fibroids do not require any treatment, particularly if they are small and not causing symptoms that are affecting your life. Your consultant will be honest with you about whether your fibroids need attention now, or whether monitoring them over time is the right approach. There is no pressure to pursue treatment unless it is genuinely in your interest to do so.
Most women with fibroids conceive naturally and have completely healthy pregnancies without any intervention. Whether fibroids are likely to affect your fertility depends on their size, number and position within the uterus. Your consultant will assess your situation carefully and give you a clear, honest answer about whether treatment is advisable before you try to conceive.
A myomectomy removes the fibroids while leaving the uterus in place. It is the preferred surgical option for any woman who wants to preserve her fertility or simply does not want to have her uterus removed. A hysterectomy removes the entire uterus and is a permanent solution, usually only considered when other options have not been successful or are not clinically appropriate. Our consultants will always explore all uterine-preserving options first.

This depends on the treatment. A hysterectomy is the only permanent solution, as fibroids cannot return without a uterus. After a myomectomy or uterine artery embolisation there is a small chance of new fibroids developing over time. Your consultant will talk you through the likelihood of recurrence and how your condition will be monitored going forward as part of your personalised care plan.
No. You can book directly as a self-pay patient without a GP letter or referral. If you have private health insurance, your insurer may ask for a GP referral letter before authorising cover, but you are welcome to call us first and we will walk you through the process. Many of our patients come to us directly, without any referral at all.
