Understanding Fibroids – Expert Care at Ovara Health

Fibroids, also known as uterine fibroids or leiomyomas, are non-cancerous growths that develop in or around the womb (uterus). They are made up of muscle and fibrous tissue and can vary in size, from small, pea-sized nodules to larger growths that can distort the uterus. While fibroids are common and often do not cause symptoms, some women experience heavy periods, pelvic pain, and complications with fertility or pregnancy.

Types of Fibroids

Fibroids can grow in different areas of the uterus and are classified based on their location:

  • Intramural fibroids – Develop within the muscular wall of the uterus and are the most common type.
  • Subserosal fibroids – Grow on the outside of the uterus and can sometimes press against surrounding organs.
  • Submucosal fibroids – Form inside the uterine lining and may cause heavy bleeding and fertility issues.
  • Pedunculated fibroids – Grow on a stalk inside or outside the uterus, sometimes leading to discomfort or pain.

Symptoms of Fibroids

Many women with fibroids experience no symptoms, but when symptoms do occur, they may include:

  • Heavy or prolonged periods, sometimes leading to anaemia
  • Pelvic pain or pressure, particularly if fibroids grow large
  • Frequent urination, if fibroids press against the bladder
  • Constipation or bloating, due to pressure on the bowel
  • Pain or discomfort during intercourse
  • Lower back pain
  • Difficulty conceiving or pregnancy complications, depending on the size and location of the fibroids

What Causes Fibroids?

The exact cause of fibroids is unknown, but research suggests they are influenced by hormonal changes, genetics, and lifestyle factors. Oestrogen and progesterone, the hormones that regulate the menstrual cycle, are thought to contribute to fibroid growth, which is why they often shrink after menopause when hormone levels decline.

How Are Fibroids Diagnosed?

At Ovara Health, we use advanced diagnostic tools to confirm the presence of fibroids and assess their size and location. Diagnosis typically involves:

  • Pelvic ultrasound scans – The first-line imaging test to detect fibroids
  • MRI scans – Provides a detailed view of fibroid size and positioning
  • Hysteroscopy – A small camera inserted into the womb to examine fibroids inside the uterine cavity
  • Laparoscopy – A minimally invasive procedure to assess fibroids located on the outer wall of the uterus

Treatment Options at Ovara Health

Fibroid treatment depends on symptoms, fibroid size, and your future fertility plans. Some fibroids do not require treatment, but if they cause discomfort or complications, several options are available:

For mild symptoms, treatment may involve pain relief medication or hormonal therapy, such as the contraceptive pill, progesterone-based treatments, or intrauterine devices (IUDs) to manage heavy bleeding.

For moderate to severe symptoms, minimally invasive procedures such as uterine artery embolisation (UAE) can be used to shrink fibroids by cutting off their blood supply. Another option is myomectomy, a surgical procedure that removes fibroids while preserving the uterus, which is ideal for women who wish to conceive in the future.

For very large or multiple fibroids, hysterectomy (removal of the womb) may be considered in cases where other treatments are not effective or if the symptoms significantly impact quality of life.
